Obtain the notice for your office

Your dental office's Notice of Privacy Practices explains how it uses and discloses protected health information, its responsibilities and your rights. Ask the Iconic Smiles office that provides your care for its current notice and the contact responsible for privacy questions.

This page provides access instructions and general information about privacy rights. It is not the practice-approved notice for all three offices and does not replace the notice supplied by your treating office. The website Privacy Policy addresses website information separately.

Requests about your dental records

Contact your treating office to ask about access to records, a correction, confidential communications or a copy of its notice. The office can explain its request process, any identification needed and how to send information securely. Please do not attach records or identity documents to an ordinary website enquiry.

Under HIPAA, covered providers must explain the uses and disclosures permitted by the Privacy Rule and when an authorization is needed. They must also describe their privacy responsibilities, patient rights and complaint contacts in their notice. The U.S. Department of Health and Human Services explains these notices.

Questions about use or sharing

Ask your office how a particular request or disclosure applies to your records. The answer may depend on the circumstances and applicable law. Information from a website assistant or a general contact response is not a substitute for reviewing the office's notice or speaking with its privacy contact.

If you have received an authorization form, ask the office to explain what it permits and how to withdraw it where applicable. An acknowledgement that you received a privacy notice is different from authorization for a special use of your records.

Raising a concern

You can raise a privacy concern with your treating office. You can also find information about complaints through the HHS Office for Civil Rights. Use the office or agency's appropriate process for sensitive records, rather than including those records in public messages or reviews.
