The general contact form asks for your first and last name, email, phone number and message. The callback form asks for your name, phone number and an optional message. When you submit, the website sends your enquiry to the connected service for the office you selected. A successful submission does not confirm an appointment or that the team has reviewed your message. The consent shown on these forms concerns a response to your enquiry; it expressly excludes marketing and text-message consent.
The website opens a chat service for the office you select. Appointment, insurance and second-opinion links open that office's dedicated intake service. Read the privacy information and any consent presented within the service before sending details or documents. Avoid including health information in a general website message.
Optional voice assistance
The website loads the software for the Bellerose AI voice guide when the page opens. A voice conversation starts when you choose its button and grant microphone access. You can end the conversation and manage microphone permission in your browser. Do not share medical records, insurance identifiers or other sensitive health details with the guide. You can use the office phone or ordinary contact options without starting it.
